Position Type: Full Time Hourly
Salary Range: $26.00 - $28.00 per hour
Schedule: Tue, Wed, Thurs, Fri (1PM - 9PM) | Saturday (8AM - 4:30PM)
Overview
Customer service is the name of the game for the Admissions Coordinator, who plays a pivotal role in day‑to‑day office operations. Responsibilities include triaging incoming calls from patients and families, initiating the referral process, verifying insurance, and keeping the Executive Director informed of daily activities.
Responsibilities
- Promote an environment that facilitates quality care, customer service, and cooperation among home care team members.
- Act as liaison between private, HMO, or federally funded insurance companies and the agency.
- Triage incoming calls related to patients, families, physicians and staff.
- Initiate referral process as outlined in department specific procedure.
- Act as liaison for hospital case managers to provide resource information and coordinate services.
- Accept patient referrals from medical office staff, physicians, hospital discharge planners, and private individuals.
- Verify MD licensure.
- Keep the Executive Director informed of day‑to‑day activities, insurance case issues, and patient care concerns.
- Work with the Executive Director to ensure agency compliance with all regulatory, licensing, and accreditation requirements.
- Actively participate in performance improvement activities as directed by the Executive Director.
- Collect data from clinical records and field staff for insurance verification, staff treatment plans, and services needed by patients.
- Assist with new‑hire orientation.
- Co‑facilitate IDT with the Executive Director as needed to ensure flow of process and documentation.
- Monitor census and track homecare admissions, discharges, certification and re‑certification dates for all patients, providing this information to IDT.
- Coordinate OASIS transmission to the state in accordance with regulatory guidelines.
- Participate in and assist with community activities such as health fairs, conventions, and community education programs as assigned by the Executive Director.
- Perform all other duties as assigned.

Requirements
- One (1) year hospice experience.
- Two (2) years of admissions experience.
- Must be at least 18 years old.
- PR certification preferred.
