Position Summary

The Addiction Counselor provides direct clinical services to individuals diagnosed with substance use disorders. This role includes assessment, treatment planning, individual and group counseling, and care coordination. The counselor works collaboratively with a multidisciplinary team to support recovery, improve functioning, and promote long‑term wellness.

Essential Duties & Responsibilities

Clinical Services

• Conduct comprehensive assessments (biopsychosocial and ASAM level of care determinations)
• Develop and implement individualized treatment plans
• Provide individual counseling sessions based on medical necessity
• Facilitate group therapy sessions (IOP and OP), including:
  • Psychoeducation
  • Relapse prevention
  • Coping and life skills

Care Coordination

• Collaborate with therapists, PRP staff, medical providers, and care team members
• Coordinate referrals to detox, higher levels of care, and community resources
• Support discharge planning and aftercare services

Documentation & Compliance

• Complete progress notes within 24–48 hours of service delivery
• Maintain documentation in compliance with:
  • CARF standards
  • Maryland COMAR regulations
