St. Luke's University Health Network is seeking an Addictions Care Manager to perform assessments, triage, and one-to-one client work within the agency and community partners. The role requires strong clinical skills in AOD services and the ability to coordinate care across levels of treatment.
Responsibilities include home visits, client meetings, drug screening, and ensuring authorization and billing for services. A valid driver's license and DDAP-aligned credentials are preferred.

Hours: Full-Time, 40 hours/week The Addictions Care Manager provides assessments and one-to-one client time and performs triage work for clients to all levels of care-both within the agency and outside agencies.
