Job summary

Qualifications

• Requires Pharmacy Technician licensure and active Pharmacy Technician certification OR Pharmacy Intern licensure in the state of practice.
• AZ Board of Pharmacy Wallet Card must be provided at time of hire.
• Two years pharmacy technician experience; knowledge of brand/generic names, OTCs and herbal supplements.
• Ability to perform moderate to difficult calculations with a handheld calculator; strong attention to detail.
• Strong communication skills, customer service focus, and ability to multi-task.

Responsibilities

• Obtain and document complete medication history by interviewing patients, families, and sources.
• Interview patients to collect prescription, OTC, herbal and supplement data for accurate history.
• Document history and allergies in the EHR; notify staff of completion.
• Follow infection control and safe interview procedures with proper identifiers.
• Maintain a safe environment; perform quality checks and unit inspections as required.
• Assist as Acute Pharmacy Technician with medications prep, charging, restocking and IV/chemotherapy where appropriate.

As an Acute Medication History Pharmacy Technician, this role obtains and documents accurate medication histories by interviewing patients, families, and other sources. Responsibilities include collecting complete information on prescription medications, over-the-counter products, vitamins, supplements, and herbal remedies to support safe and effective patient care.

The schedule for this position is four-10 hour shifts including Saturday and Sunday and every other weekend. Hire On Incentive up to $2,500 available. Recruiter to provide these details during the interview process. Enjoy a flat rate $1/hour weekend shift differential and an 18%-night shift differential when applicable.

POSITION SUMMARY

This position, under the supervision of a pharmacist, is responsible for obtaining a patient’s complete medication history through communication with the patient, family, and/or other resources and documenting it into the medical record. Medication History technicians (MHTs) perform an important function in collecting an accurate and complete medication history from every patient they encounter. A complete medication history will include prescription medications, over-the-counter (OTC) products, herbal supplements, vitamins/minerals, and other similar products.

CORE FUNCTIONS

1. Identifies and prioritizes patients according to need for medication history collection. Reviews and collects proper information for interview prior to patient interaction.

2. Interviews patients to collect the medication history. Asks pertinent questions to ensure capture of all prescription medications, over-the-counter medications, and herbal/dietary supplements. As necessary, contacts family or other resources (e.g. outside pharmacy) to get a more complete medication history.

3. Documents medication history and allergies in the electronic health record according to established procedures. Tracks history collection through intervention documentation. As appropriate, notifies proper hospital staff of medication history completion.

4. Follows procedure for infection control, safe patient practices (e.g. using 2 patient identifiers), interpreter services, and contraindications to patient interview.

5. Maintains a safe and clean working environment by complying with all policies, laws, and regulations. Performs quality control activities such as monthly nursing unit inspections and pharmacy section checks in a timely manner.

6. Assumes the duties of the Acute Pharmacy Technician when needed and as appropriate. These duties may include, but are not limited to preparing, charging and crediting medications, restocking automated dispensing technology and compounding specialty medications and intravenous medications, to include hazardous and chemotherapeutic agents.

MINIMUM QUALIFICATIONS

Requires Pharmacy Technician licensure and active Pharmacy Technician certification OR Pharmacy Intern licensure in the state of practice.

Incumbents in Arizona are also required to provide the AZ Board of Pharmacy Wallet Card at time of hire.

Minimum of two years pharmacy technician experience. Must have the knowledge of brand/generic names of drugs, including OTC’s and their strengths/uses, as well as knowledge of herbal/dietary supplements and their strengths/uses. Ability to complete moderate to difficult mathematical calculations using a hand‑held calculator is required. Strong customer service focus, exceptional communication skills, strong attention to detail, prioritization capabilities, superior problem-solving skills, and ability to multi-task. Must have knowledge of computer software, particularly medication management information system software, Microsoft applications and Automated dispensing systems. Must demonstrate effective oral and written communication skills for interfacing with all levels of staff, physicians, patients, and other contacts.

Employees working at Banner Behavioral Health Hospital must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. An Arizona Criminal History Affidavit must be signed upon hire.

PREFERRED QUALIFICATIONS

Previous community pharmacy technician experiences.

Additional related education and/or experience preferred.
