Job summary

Elevance Health is seeking Advanced Practice Providers (Nurse Practitioners) who are licensed in three or more states and hold an active RN Compact license. This virtual eligible role requires an MS in Nursing, NP certification, and a valid NP license in applicable states.

The position involves delivering primary and urgent care via telehealth, developing care plans for adults with chronic conditions, and coordinating with an inter-disciplinary team while documenting in EMR.

Qualifications

• Requires an MS in Nursing.
• Requires an active national NP certification.
• Requires valid, current, active and unrestricted Family or Adult Nurse Practitioner license in applicable states.
• Requires 2+ years of experience in managing complex care cases.
• Experience working with EMR systems.

Responsibilities

• Provides primary and urgent health care via telephone and tele video modalities to Medicaid patients.
• Develops and implements clinical plans of care for adults with chronic/complex conditions.
• Gathers history, performs exams, and creates treatment plans aligned with patient goals.
• Educates patients about medications, adherence, and prevention.
• Maintains medical records per state regulations and company policy.
• Collaborates with interdisciplinary team and other providers.
• Prescribes medication as permitted by state authority.
