Job summary

Cynet Health seeks a Social Worker with LCSW/LMSW to support acute rehabilitation patients in a 13-week assignment. You will perform initial evaluations, develop care plans, and coordinate post-discharge services for adults and geriatrics.

Strong teamwork and documentation are essential for quality, cost-effective care. Must have BLS and LCSW/LMSW credentials; travel experience and hospital exposure are preferred.

Qualifications

• Experience in acute rehabilitation and hospital settings.
• Strong discharge planning and care coordination background.
• Familiar with charting systems and interdisciplina ry rounds.

Responsibilities

• Conduct initial evaluations and develop plans of care.
• Coordinate services to home care and equipment after discharge.
• Document care delivery, goals, and patient response.
• Facilitate discharge and contingency planning with the team.
• Attend daily interdisciplinary rounds and collaborate with colleagues.
