Vibra Healthcare, LLC is seeking a Speech Pathologist for a Long Term Acute Care facility in North Carolina. The role includes evaluations, treatment planning, patient treatments, and discharge planning to improve speech, language, swallowing, cognitive, and hearing disorders.

Responsibilities cover assessments, interdisciplinary collaboration, documentation, discharge instructions, and ongoing education for patients and families. Licensure in NC and BLS certification are required.

Qualifications

• Current Speech/Language Pathology license to practice in North Carolina.
• Ability to read and interpret safety rules, operating and maintenance instructions.
• Maintains current BLS certification.

Responsabilités

• Assess and evaluate impact of available patient information on the assessment and treatment.
• Provide care for the Adult or Geriatric patient, ages 18 years and over.
• Develop and implement an individualized treatment program with the interdisciplinary team.
• Submit patient charges accurately and in a timely manner.
• Enter a progress note in the patient record following each treatment session.
• Attend interdisciplinary team conferences regarding patient progress.
