Live Well Homecare is seeking an RN for Acute Care to provide high-quality, patient-centered care on hospital acute care units in Colorado. You will assess, plan, and implement individualized care, coordinate with an interdisciplinary team, and educate patients and families on care plans and medications.

Qualifications

• Current RN licensure and ability to deliver acute care.
• Strong clinical judgment, assessment, and medication administration skills.
• Effective communication and patient/family education capabilities.
• Ability to collaborate across disciplines and maintain accurate documentation.

Responsibilities

• Perform comprehensive patient assessments and develop/monitor individualized care needs.
• Administer medications, therapies, and other prescribed treatments with safety and accuracy.
• Coordinate and collaborate with an interdisciplinary care team to support optimal outcomes.
• Educate patients and families on care plans, medications, and health management.
• Use evidence-based practice to guide clinical decisions.
• Maintain accurate electronic nursing documentation.
• Support quality improvement and patient safety initiatives.
