Responsibilities

• Assess patient needs across physical, psychological, social, spiritual, and discharge planning.
• Formulate and adjust a care plan in collaboration with the patient, family, and health team.
• Administer and coordinate care per physician orders and nursing plan of care.
• Document assessments, plans, and patient responses accurately in medical records.
• Provide teaching and support, participate in leadership and education of staff and students.
