Job summary

Temple Health is seeking an RN Case Manager at Temple Hospital in Philadelphia for a full-time Day shift. The role collaborates with patients, families, physicians and the interdisciplinary team to facilitate progression through the acute care episode and to create a timely transition plan within 48 hours of admission.

Requires a BSN or commitment to obtain BSN within 2 years, and at least 1 year of acute care Case Management or Utilization Management experience.

Qualifications

• BSN required or eligible for BSN within 2 years from date of hire
• 1 year experience in acute care Case Management or Utilization Management
• General experience and knowledge of InterQual criteria preferred

Responsibilities

• Facilitates patient progression through acute care episode in collaboration with patient/family, physicians and the inter disciplinary team
• Monitors level of care and initiates evidence-based protocols and order sets
• Influences care progression and develops a transition plan within 48 hours of admission
• Acts as advocate for patient and family throughout the acute episode of care
