CHS Career Site is seeking a PRN Physical Therapist for acute care in Indiana. The therapist evaluates patients, plans individualized treatments, delivers therapy, and reassesses progress while documenting all findings. Strong collaboration with physicians and care teams ensures quality patient outcomes.

Requirements include a Bachelor’s Degree in Physical Therapy and a current PT license. BLS certification within 30 days of hire is expected.

Responsabilités

• Conduct PT evaluations and develop individualized treatment plans.
• Administer safe, effective therapies and document interventions.
• Reassess and adjust plans based on patient progress.
• Document evaluations, plans, progress notes, and billing details.
• Educate patients and families on exercises and post-care.
• Maintain safe, clean therapy environment and equipment.
