Job summary

Essentia Health in Fargo, ND, seeks a licensed Physical Therapist to evaluate, plan, and implement care for patients across a full caseload.

You will coordinate with a multidisciplinary team to optimize clinical and financial outcomes, provide education to patients and caregivers, and manage documentation and scheduling responsibilities.

Flexibility to cover across departments, participate in leadership activities, and support patient transitions is encouraged.

Qualifications

• Bachelor's, Master's, or Doctorate in physical therapy from an accredited program.
• Current license to practice in the state.
• Ability to plan and provide therapy and discharge planning for a full caseload.

Responsibilities

• Provide therapy evaluation, develop individualized treatment plans, implement evidence-based interventions, provide patient/caregiver education, and discharge planning for a full patient caseload.
• Complete documentation, billing, scheduling management, and other administrative duties for a full caseload.
• Collaborate with a multidisciplinary care team to ensure optimal clinical and financial outcomes.
• Coordinate with referring physicians and Essentia Health leadership to ensure cohesive patient care.
• Deliver durable medical equipment to patients and mentor staff as needed.
