Qualifications

• ASHP-accredited PGY1 residency or 3 years of pharmacy experience.
• Meets HM standards of an ASHP-Qualified Residency Preceptor within 1 year.
• Proficiency in English for patient care and documentation.
• Experience with drug information, formulary processes and education of staff.

Responsibilities

• Perform advanced clinical pharmacy duties and medication reviews.
• Precept pharmacy students and residents on clinical rotations.
• Support formulary processes and medication reconciliation activities.
• Educate fellow pharmacists, nurses, and medical staff.
