UAB Medicine in Birmingham, AL is seeking a pharmacist to provide patient-focused pharmacy services, monitor therapy, educate and distribute medications. The role supports the division’s mission and hospital programs.

You will assess prescriptions, monitor patient responses, document medication histories, and collaborate with physicians to optimize therapy. Mentoring students and residents is also part of the role, with rotating every 4th weekend day shifts and PRN coverage.

Qualifications

• Bachelor’s degree in Pharmacy required; license by the Alabama Board of Pharmacy (ALBOP) required.

Responsabilités

• Accurately assesses medication appropriateness based on diagnosis, allergies, and history; reviews for adverse reactions and interactions; makes recommendations to physicians as needed.
• Monitors patient-specific responses to therapies; reevaluates therapies and suggests changes as appropriate.
• Documents medication history; notifies physicians about issues with adherence or therapy.
• Ensures proper distribution of medications to meet patient care needs; triages orders.
• Provides drug information to patients and families, physicians, pharmacists, nurses and other health care professionals; shares new information from literature.
• Supports educational programs by mentoring pharmacy students/residents.
• Performs other duties as assigned.
