Job summary

The Registered Nurse (Case Management) role is with the Providence VA Healthcare system in Providence, Rhode Island. It’s a permanent, full-time position in Nursing - Acute Care, working 40 hours per week on-site.

Responsibilities include care coordination, discharge planning, and collaboration with interdisciplinary teams to meet complex veteran health needs. Licensure and experience in case management are emphasized.

Qualifications

• English language proficiency required as per 38 U.S.C. 7403(f)
• Current, full, active nursing license in the United States or District of Columbia
• Graduate from an approved nursing program (ACEN/CCNE) or bridge program with licensure
• Foreign educated nurses require degree equivalency validation
• Valid licensure and state board eligibility for appointment as RN Case Manager

Responsibilities

• Provides patient management, care coordination and discharge planning for inpatient and outpatient settings
• Coordinates care with multiple providers across all levels of care
• Engages patients and families to integrate knowledge and values into care planning
• Assists with complex health care needs including acute, chronic, and post-acute care transitions
• Ensures timely and appropriate provision of services and outcomes evaluation
