Job summary

ArchWell Health in Arizona seeks an Acute Care Nurse Practitioner to deliver primary and acute care services across ArchWell Health centers under physician supervision. You will document plans of care, maintain patient records, and collaborate with care teams to ensure high-quality experiences for seniors.

Responsibilities include supporting RNs, coordinating with the patient’s PCP, participating in clinical meetings, and being available for call coverage as needed.

Qualifications

• Able to demonstrate interpersonal skills to be approachable and understandable to patients, families, and center team members.
• Strong understanding of examination methodologies and diagnostics.
• Comfortable traveling within the market to multiple centers within short distance of each other.

Responsibilities

• Render outpatient primary care and consulting services under physician supervision and collaboration agreements.
• Maintain accurate records of all professional services and patient encounters.
• Document care plans and ensure continuity with the patient’s Primary Care Provider.
• Collaborate with RNs and other care team members to provide timely, high-quality care.
• Be available for call coverage on nights, weekends, or holidays per ArchWell Health schedule.
