Job summary

gobravvo seeks an Acute Care Nurse Practitioner (ACNP) to provide comprehensive acute and primary care for adults and older adults in Phoenix, AZ. This full-time, on-site role emphasizes patient-centered care, collaborative teamwork, and care coordination across sick visits, urgent needs, and follow-ups.

The ACNP will diagnose, manage, and document care, work closely with physicians, RNs, MAs, and other team members, and participate in program development and quality improvement initiatives.

Qualifications

• Active NP license in Arizona.
• 1 year of primary care/geriatrics/family medicine experience.
• BLS and ACLS certifications.
• Strong clinical assessment and documentation skills.
• Experience caring for older adults with complex conditions.
• Excellent communication and teamwork.

Responsibilities

• Provide acute and primary care to adult patients (sick visits, follow-ups, walk-ins).
• Diagnose, treat, and document patient care per guidelines.
• Collaborate with physicians, RNs, MAs, and care team.
• Coordinate with primary care providers for continuity.
• Support quality improvement, patient outreach, and program development.
• Participate in after-hours/weekend rotation and travel between locations.
