Aspirus Keweenaw Hospital in Laurium, MI is seeking an Acute Care Coordinator (RN or Social Worker) to join our team. The role involves assessing patient needs and coordinating referrals to resources to meet those needs and developing discharge plans.

You will work with physicians, nursing and ancillary staff to coordinate care, exercise independent judgment, and advocate for patients while navigating federal, state, and local discharge regulations.

Aspirus Keweenaw Hospital in Laurium, MI is seeking an ACUTE CARE COORDINATOR (RN or SOCIAL WORKER) to join our team!

The Acute Care Coordinator is responsible for assessing patient needs and coordinating referrals to resources to meet those needs. They use knowledge in the development of the patient's plan of care and attempt to resolve social, economic, and environmental difficulties that interfere with the patients' ability to manage their health needs effectively. As part of the multidisciplinary team, the Acute Care Coordinator works closely with physicians, nursing, and ancillary departments, and provides support and clinical expertise to nursing personnel regarding patient care coordination issues. This position requires independent judgment, analytical, problem solving, and collaborative skills in order to organize and prioritize multiple tasks to provide patient centered care.

PROFESSIONAL EXPERIENCE/QUALIFICATIONS

• Knowledge and skills normally acquired through completion of a degree in nursing (ADN or BSN) OR social work (BSW or MSW) preferred.
• Approximately 6 months to one year of on the job experience necessary in order to gain full knowledge of federal, state, and local regulations related to discharge planning and knowledge of health and welfare resources and facilities.
• Exhibits strong human relation skills for interacting with patients, patient's family members, physicians, personnel, community agencies, and other resources.
• Displays analytical skills in order to assess patients' needs, develop associated discharge planning, and provide sound recommendations and guidance. Displays ability to concentrate and pay close attention to detail when interviewing patients & preparing discharge plans.
• Possesses planning and organizational skills and expert skills in prioritization.
• Understands the nature of social diversity and oppression with respect to race, ethnicity, national origin, color, sex, sexual orientation, gender identity or expression, age, marital status, political belief, religion, immigration status, and mental or physical disability.

LICENSURE

• Possesses current Michigan Social Work or RN license

HOURS: Full Time; 1.0 FTE or 80 hours per pay period.
