Job summary

UofL Health University Hospital - ULP - CMG in Louisville, KY offers a 12-month Advanced Practice Provider Fellowship designed to prepare fellows for the independent and fast-paced role of an APP. The program combines core rotations with elective time, didactics, precepted practice, and autonomous clinical skills development to build a strong foundation for patient care.

Responsibilities

• Provide direct patient care to an assigned load of patients determined by the service assigned.
• Perform comprehensive relevant health, social, medical history and thorough physical assessments.
• Identify and evaluate health and social disparities influencing a patient’s health and well-being.
• Diagnose and develop a plan of care considering cost, health equity, and diversity.
• Identify priorities to satisfy the needs of the patient, family, and community.
• Order and interpret labs and tests, while reducing waste and maximizing benefit.
• Monitor effects of medications and therapies.
• Review documentation and participate in rounds with the physician team.
