Job summary

Qualifications

Responsibilities

• Plan, organize, conduct, and ensure implementation of Activity Therapy programs such as leisure activities, arts/crafts, and community outings.
• Collaborate with unit Treatment Team to set individualized goals and communicate patient progress.
• Maintain required records and clinical documentation to meet accrediting/certifying standards (Medicare/Medicaid & Joint Commission).
• Transport and escort clients for community activities per treatment plans; track attendance and outcomes.
• Follow agency policies, procedures, and safety practices.
