Job summary

Waters of Clifty Falls in Indiana seeks an Activities Director to plan, organize and direct the Activities Department, ensuring an on-going program designed to meet residents’ physical, mental and psychosocial well-being.

You will facilitate group activities, encourage participation, assist with 1 on 1 activities and coordinate outings, while collaborating with all disciplines to deliver patient-centered care. This is a full-time, on-site role with health, retirement and PTO benefits.

Qualifications

• High School diploma or equivalent.
• 3 years experience in Activities Department in a Skilled Nursing Facility.

Responsibilities

• Plan, organize, develop and direct the overall operation of the Activity Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator, to assure that an on-going program of activities is designed to meet, in accordance with the comprehensive assessment, the interests and the physical, mental and psychosocial well‑being of each resident.
• Facilitate group activities
• Encourage activity involvement and chart attendance
• Assist in keeping activity supplies clean and organized
• Assist with 1 on 1 activities, transport residents to and from activities
• Assist with outings and special events and other assigned duties
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
