Job summary

Job description

This is a part time position. Mainly Saturdays and Sundays.

SNF or similar experience preferred.

POSITION SUMMARY

The purpose of your job position is to assist in the planning and implementation of activities as they meet the needs of the resident care plans in accordance with current federal, state and local standards that govern the facility, and as directed by the Director of Activities.

The Activity Assistant will treat each resident fairly, kindly, and show them dignity and respect. They will knock on the resident’s door before entering. They will greet them upon entering their room. They will refer to each resident by their name. They will communicate clearly, empathetically, and effectively when speaking to residents, family members, visitors, all staff and governmental agencies. They will work cooperatively with all departments and multidisciplinary teams. They will demonstrate patience, initiative and willingness to assist residents that may be difficult or in discomfort. They will relate all pertinent information concerning a resident’s condition to a charge nurse as required. They will be committed to always doing the right thing.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Planning, organizing and conducting activities for the facility
  • Involving residents and family members in the planning whenever possible
  • Arranging transportation for field trips when necessary
  • Ensuring all charted activity progress notes are descriptive and detailed
  • Coordinating activities with other departments and working collaboratively with all staff
  • Following Infection and Control policies and Safety and Sanitation procedures
  • Reporting any occupational exposures to blood, body fluids, or other hazardous materials to a supervisor immediately
  • Protecting each residents file and data with confidentially per HIPAA standards
  • Abiding with all facility policies and procedures including not disclosing user ID codes and passwords

REQUIREMENTS
Education / Licensure
  • High school diploma or GED
Qualifications / Experience
  • Minimum of 6 months experience in skilled nursing preferred
  • Must be able to read, write, speak, and understand the English language
  • Must be able to follow directions
Working Conditions
  • May encounter frequent interruptions
  • May be involved with residents, family and government agencies
  • May be requested to work beyond scheduled working hours at times
  • May be exposed to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses
Physical Requirements
  • Must be able to move intermittently throughout the day
  • Repetitive hand motion
  • Must be able to push, pull, move and or lift up to 25 lbs. to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 15 feet
