Job summary

Laguna Hills Health and Rehabilitation is seeking an Activity Assistant to help plan and implement resident activities in line with care plans and regulatory standards. You will support dignified interactions, greet residents, respect privacy, and coordinate with nurses and staff to ensure safety and quality of life.

Responsibilities

  • Plan, organize and conduct activities for the facility.
  • Involve residents and family members in planning when possible.
  • Arrange transportation for field trips when necessary.
  • Ensure all charted activity progress notes are descriptive and detailed.
  • Coordinate activities with other departments and work collaboratively with all staff.
  • Follow Infection and Control policies and Safety and Sanitation procedures.
  • Report any occupational exposures to a supervisor immediately.
  • Protect residents' files and HIPAA confidentiality.
  • Abide by facility policies including not disclosing user IDs and passwords.

POSITION SUMMARY

The purpose of your job position is to assist in the planning and implementation of activities as they meet the needs of the resident care plans in accordance with current federal, state and local standards that govern the facility, and as directed by the Director of Activities.


The Activity Assistant will treat each resident fairly, kindly, and show them dignity and respect. They will knock on the resident’s door before entering. They will greet them upon entering their room. They will refer to each resident by their name. They will communicate clearly, empathetically, and effectively when speaking to residents, family members, visitors, all staff and governmental agencies. They will work cooperatively with all departments and multidisciplinary teams. They will demonstrate patience, initiative and willingness to assist residents that may be difficult or in discomfort. They will relate all pertinent information concerning a resident’s condition to a charge nurse as required. They will be committed to always doing the right thing.


ESSENTIAL DUTIES AND RESPONSIBILITIES


  • Planning, organizing and conducting activities for the facility

  • Coordinating activities with other departments and working collaboratively with all staff

  • Reporting any occupational exposures to blood, body fluids, or other hazardous materials to a supervisor immediately


  • Protecting each residents file and data with confidentially per HIPAA standards


  • Abiding with all facility policies and procedures including not disclosing user ID codes and passwords

REQUIREMENTS

Education / Licensure


  • High school diploma or GED



Qualifications / Experience


  • Minimum of 6 months experience in skilled nursing preferred


  • Must be able to read, write, speak, and understand the English language


  • Must be able to follow directions



Working Conditions


  • May encounter frequent interruptions


  • May be involved with residents, family and government agencies


  • May be requested to work beyond scheduled working hours at times


  • May be exposed to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses



Physical Requirements


  • Must be able to move intermittently throughout the day


  • Repetitive hand motion


  • Must be able to push, pull, move and or lift up to 25 lbs. to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 15 feet


  • Ability to read fine prints
