Fair Oaks Health and Rehab in Jamestown, KY seeks an Activity Assistant to brighten residents' days through engaging programs and meaningful interactions. You will plan group activities, support one-on-one time, and help create a warm, inclusive atmosphere where residents feel valued and connected.

Responsibilities include coordinating activities, encouraging participation, supervising outings, and assisting with registrations while maintaining supplies and adhering to budgets.

Qualifications

• CNA license required.
• High school diploma or GED required or equivalent related work experience.
• 6–12 months experience in activity-related duties or similar.

Responsibilities

• Assist in planning, coordinating, and evaluating resident activities.
• Encourage resident participation in programs and activities.
• Escort residents on activities away from the property as assigned.
• Assist with resident registrations for trips and programs.
• Conduct activities as directed by the Resident Program Director.
• Maintain activity supplies and possibly handle outside purchasing.
• Survey and interview residents to determine interests and satisfaction.
• Maintain observation of residents and share information with staff.
• Follow departmental budget guidelines.
• Attend required training, in-service and staff meetings.
• Promote safety and professional demeanor in interactions with residents and staff.
• Adhere to all company policies and procedures.
• Performs other duties as assigned.

Essential Functions

• Assists in planning, coordinating, and evaluating resident activities.
• Encourages resident participation in programs and activities.
• Escorts residents on activities away from the property as assigned.
• Assists with the registration of residents for trips and programs.
• Conducts various activities at the direction of the Resident Program Director.
• Maintains necessary activity supplies, which may include outside purchasing.
• Assists in the surveying and interviewing of residents to determine interests, talents and resident satisfaction.
• Maintains general observation of residents and shares appropriate information with staff.
• Follows departmental budget guidelines.
• Attends all required training, in‑service, and staff meetings.
• Strives to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.
• Maintains a positive and professional demeanor toward residents, visitors, families, and co‑workers.
• Adheres to all Company policies and procedures
• Performs other duties as assigned

Requirements

• CNA license
• High school diploma or GED required or equivalent related work experience.
• Six (6) months to one (1) year experience or related experience performing activity related duties
• Ability to assume leadership role in the activities department in the absence of the Activities Director
• Skill at working with individuals who have cognitive, physical or sensory disabilities

Moderate physical activity

• Requires handling of average‑weight objects up to twenty five (25) pounds.
• Repetitive standing and/or walking for more than four (4) hours per day.
• Repetitive bending and/or stooping for more than one (1) hour at a time.
• Sitting for more than two (2) hours at a time.
• May require consistent computer work with repetitive typing and concentrating on computer screen.
• Physical ability to travel to sites.
