Job summary

Lemont Nursing and Rehab in Lemont, IL is seeking a dedicated Activity Aide to enhance the daily living experience for residents. You will lead engaging group and individual activities, guided by assessments and care plans, to foster social interaction and meaningful engagement.

The role requires 1-2 years in a social or recreational program within a healthcare setting, and offers a comprehensive benefits package for full-time team members.

Qualifications

• 1 - 2 years of experience in a social or recreational program in a healthcare setting (preferred).

Responsibilities

• Facilitate ongoing group and individual activity programs based on residents’ needs, interests, and abilities, as identified in assessments and care plans.
• Complete required record-keeping upon resident admission, discharge, and transfer; chart and document per facility policy.
• Assist in creating the activity calendar and coordinating events.
• Follow and maintain established policies and procedures.
• Encourage resident participation in Resident Council and schedule monthly meetings.
