Job summary

Lemont Nursing and Rehabilitation Center is seeking a dedicated Activity Aide to enrich resident daily living through engaging activities and meaningful interaction. The role focuses on creating social, enjoyable experiences aligned with residents' needs and interests.

As part of our Extended Care community, you will facilitate group and individual activities, maintain records, help plan calendars, and encourage resident participation in council meetings while upholding facility policies.

Qualifications

• 1-2 years of experience in a social or recreational program in a healthcare setting (preferred).
• Ability to facilitate group and individual activities based on residents’ needs and interests.
• Experience documenting activities and following facility policies.

Responsibilities

• Facilitate ongoing group and individual activity programs based on residents’ needs, interests, and abilities, as identified in assessments and care plans.
• Complete required record-keeping upon resident admission, discharge, and transfer; chart and document per facility policy.
• Assist in creating the activity calendar and coordinating events.
• Follow and maintain established policies and procedures.
• Encourage resident participation in Resident Council and schedule monthly meetings.

Activity Aide Requirements:
