Job summary

Seven Hills Foundation is seeking an Activities Specialist to support individuals with disabilities in daily living activities and engage them in meaningful programs. The role focuses on personal care, environment safety, and collaborative planning with clinical teams to sustain progress and well-being.

You will lead diverse activities, monitor progress, and document results while helping participants access transportation to events and appointments.

Qualifications

• High School diploma and one year related experience are required.
• Valid driver’s license (class C or D1) and good driving record.
• Ability to support personal care needs with dignity and respect.

Responsibilities

• Assist individuals with dining, toileting, and transfers, ensuring comfort and dignity.
• Develop and lead individual and group activities to promote skill development and independence.
• Collaborate with clinical team to implement behavior plans tailored to each individual.
• Assist with transporting participants to activities and appointments as needed.
• Evaluate and document participant progress and ISPs compliance.
