Job Description

At Liberty Healthcare and Rehabilitation Services, we promote a challenging but rewarding environment. We are currently seeking an Activity Assistant for a full-time, day shift (10 am - 6 pm, Saturdays).

1. Assist in planning, promoting, and leading individual and group activities based on residents' needs and interests.
2. Encourage social communication through group activities.
3. Assist residents with participation in religious programs and leisure activities.
4. Help with the monthly newsletter and activities calendar.
5. Maintain, store, and use activities supplies properly.
6. Support the volunteer program and appreciation events.
7. Transport residents to and from activity programs.
8. Assist with admissions assessments, clinical responsibilities, MDS, care plans, and progress notes according to policies.
9. Facilitate community interaction programs.

Job Requirements

1. High school diploma or equivalent.
2. Experience as Activities Assistant or in Long Term Care/Memory Care/Assisted Living preferred.
3. Certification as an Activities Professional preferred.
4. Ability to work well with different personalities and collaborate with staff.
5. Positive, energetic, and professional attitude.
6. Self-motivated and capable of working independently.
7. Proficient in reading, writing, and understanding English.
8. Ability to work under pressure and manage supplies economically.
9. Valid Driver’s License; CDL preferred or ability to obtain one.
