Responsibilities include planning and conducting activities for individuals, small groups, and large groups. Assist in communication between staff, residents, families, and external agencies to meet residents' needs. Participate in community planning and develop monthly activity calendars, maintaining attendance records. Support quality assurance initiatives, discharge planning, and resident assessments. Arrange transportation for outings and appointments. Encourage residents to participate in hobbies, crafts, and reading, providing necessary materials, including Braille or audio books. Assist with documentation and keep the activity area organized.
Preferred qualifications include at least one year of experience in a long-term care facility. Skills required include reading and comprehending policies, effective communication, basic math concepts, problem-solving, and interpreting various instructions. Physical demands involve frequent standing, walking, reaching, talking, hearing, and lifting up to 50 pounds. The work environment is typically low to moderate noise, with accommodations available for individuals with disabilities.
