JOB PURPOSE:
Assists in the planning, implementation, and evaluation of recreational, social, intellectual, emotional, and spiritual programs, in accordance with the patient's assessment and care plan, and as directed by the Supervisor.
KEY RESPONSIBILITIES:
- Conduct assessments and gather information by interviewing residents, interested family members, legal representatives, and significant others to develop individualized recreation programs that accommodate the functional levels, needs, and interests of each resident.
- Update resident recreation programs as needed to accommodate individual needs and preferences and to protect and promote residents’ rights.
- Work cooperatively with members of the interdisciplinary team to develop, implement, and evaluate the care plan. Attend care conferences and develop activity interventions as part of the comprehensive plan of care. Communicate resident concerns and responses to interventions to team members and staff.
- Schedule recreational activities to accommodate residents' hours (morning, afternoon, evening, weekend), appealing to all age groups, in various locations, including seasonal events. Post monthly activities calendar.
- Provide activities to residents confined to their rooms, reflecting lifelong interests such as music, reading, visits, and in-room projects, which residents can work on independently or with staff assistance.
- Offer structured activities for cognitively impaired residents, such as music events, gross motor games, or outdoor walks. Adapt activities for participation, using segmentation, verbal prompts, physical support, demonstrations, and hand-over-hand assistance as needed.
- Develop and implement procedures to ensure residents are informed about activities, staff encourages participation, and transportation is provided.
- Record progress notes in the clinical record, including subjective and objective findings, behavior observations, interventions, and resident responses. Audit chart entries for completeness and accuracy.
- Perform quality assessment and assurance functions, including compliance rounds, activity observations, record reviews, and interviews. Identify staff performance issues and institute corrective actions.
- Develop and update recreation policies and procedures aligned with the facility's philosophy, standards, and regulations.
- Communicate daily with the Nursing Home Administrator regarding resident care, staffing, and budget issues. Develop staffing plans and recommend personnel needs to meet resident requirements.
- Participate in developing the department budget, providing relevant financial information to the Administrator.
MINIMUM EDUCATION REQUIRED:
Minimum two (2) years of college education with courses in recreation, education, social work, behavioral sciences, or clinical services.
MINIMUM EXPERIENCE REQUIRED:
Two (2) years of experience in a social or recreational program within the past five years, including one year of full-time employment in a healthcare setting with patient/resident activities.
