Job summary

Southwest Network seeks a dedicated ACT case manager to deliver assertive treatment services in the community. You will implement treatment plans, coordinate ISPs, advocate for members, and assist with crisis intervention and discharge planning.

This role requires strong communication, organization, and software skills, plus engagement in after-hours coverage. Training aligns with the Nine Guiding Principles for Recovery Oriented Services.

Qualifications

  • Master's or bachelor's degree in behavioral health or related field or alternative combinations with related experience.
  • Minimum 1 year working with seriously mentally ill adults; ACT team experience preferred.
  • Certifications: CPR, First Aid, Therapeutic Options; valid Arizona driver license and car insurance.

Responsibilities

  • Provide assertive community treatment services and implement treatment, rehabilitation, and support plans.
  • Develop ISPs and coordinate services related to ISPs; advocate for members and coordinate discharge plans.
  • Provide crisis intervention and after-hours coverage; participate in team activities and ensure member safety.
  • Maintain documentation and communication to support recovery-oriented services and outcomes.
  • Adhere to Nine Guiding Principles for Recovery Oriented Adult Behavioral Health Services.

Job description

Description

As a member of an Assertive Community Treatment (ACT) team, functions as a case manager with primary responsibility for providing assertive treatment services, implementing plans for treatment, rehabilitation and support services. Coordinates, monitors, and provides direct services to members in a community setting. Participates in the development of individualized service plans (ISPs) and provides/coordinates services related to ISPs. Acts as a member advocate, coordinates services and discharge plans, and assists in crisis intervention. Provides after-hours care, on-call coverage, and support to all members being served by the ACT team.


All members of our team are expected to provide services in accordance with the Nine Guiding Principles for Recovery Oriented Adult Behavioral Health Services and Systems.

REQUIREMENTS

Education, Certification, and Experience Requirements


  • Must possess one of the following:

    • Master's or bachelor's degree with a behavioral health or related field

    • Bachelor's degree in a non-related field PLUS one year related experience

    • Associate's degree in any field PLUS one year related experience

    • High school diploma or equivalency certificate PLUS three years' related experience



  • Minimum (1) one year working with seriously mentally ill adults required; previous experience working on an ACT team or equivalent experience preferred.

  • Must maintain current CPR, First Aid, and Therapeutic Options certifications (offered through Southwest Network)

  • Must maintain a valid Arizona driver's license and car insurance

  • Must be at least 21 years of age.



Knowledge, Ability, and Skills


  • Must have the ability to maintain excellent member and family relationships and provide excellent customer service

  • Requires effective written, verbal and interpersonal communication skills

  • Ability to stay organized and have critical thinking, time management and problem solving skills are a must

  • Ability to utilize multiple types of office equipment (i.e. phones, copier, fax, etc.)

  • Ability to type 25 to 40 WPM is preferred

  • Must be proficient in Microsoft Office software
