Job summary

Responsibilities

Job Overview: The ACT Team Registered Nurse functions as an integral part of the Assertive Community Treatment Team to provide intensive, recovery-based treatment for the highest risk consumers of mental health resources. The ACT Team Registered Nurse manages the medication system; documents medication treatment; communicates and coordinates with medical providers; engages in health promotion, education, and provider training to other team members; and assists consumers with understanding medications prescribed and the importance of taking medication.

Responsibilities and Duties:

• Work collaboratively as part of the ACT Team in order to provide comprehensive care that meets the individual needs of each client enrolled in ACT;
• Assess client health daily and respond appropriately to any noted concerns (i.e. medication side effects, withdrawal symptoms, general health changes, etc.);
• Monitor client use of medications, changes in medications, and assess for efficacy, safety, and/or side effects of medications;
• Collaborate with all providers involved with client’s health care and provide feedback to providers as needed;
• Link client to appropriate healthcare providers or specialists and follow-up with all referrals and recommendations;
• Provide health and wellness education to the client, the client’s supports, and other ACT Team staff members;
• Participate in the formal training and development process with administrative staff and consultants as well as ongoing professional development and continuing education;
• Attend daily team meetings, individual supervision, and group supervision as scheduled;
• Initiate and maintain relationships with community resources, outside human service agencies, and other stakeholders as needed;
• Commit to ensuring fidelity to the ACT model and ongoing quality improvement efforts;
• Complete all required documentation accurately and on time;
• Knowledge, understanding, and experience providing mental health and substance use treatment, including experience conducting mental health evaluations, familiarity with psychiatric medications, and awareness of medication assisted treatment (MAT) practices;

Education and Experience:

Candidates must have the following qualifications:

• Registered Nurse, licensed to practice without limitations in Pennsylvania
• Experience working in a behavioral health setting preferred
