Qualifications

  • Master's degree in psychology, social work, or related human services field.
  • 2 years’ experience in community mental health preferred.
  • Valid driver’s license.

Responsibilities

  • Participate as a member of the Assertive Community Treatment multidisciplinary team.
  • Provide direct clinical services including individual supportive therapy and psychotherapy.
  • Provide individual and group treatment in a stage-based treatment model.
  • Develop individualized treatment plans with patient-centered goals.
  • Develop crisis plans to support community-based management during crises.
  • Coordinate treatment plans to maximize impact of services.
  • Complete screenings and intakes for ACT referrals.
  • Assess for involuntary admissions when designated by the psychiatrist.
  • Refer and coordinate with outside services and supports for continuity of care.
  • Deliver treatment options including therapy, functional support services and case management.
  • Participate in supervision and daily team meetings to ensure communication.
  • Provide on-call crisis intervention during night shifts and weekends.

Position Summary:

Assertive Community Treatment (ACT) is a patient-centered, recovery oriented, mental health service delivery system. The ACT Clinician provides community based psychosocial rehabilitation services for individuals with the most severe mental illness. The ACT Clinician plans, coordinates, provides and monitors therapeutic, rehabilitative, support and community integration services to assist individuals to integrate as independently and fully into the community as possible. The ACT Clinician works collaboratively as part of a multidisciplinary treatment team, and must demonstrate excellent clinical judgment, decision making and safety consciousness to assure safety and appropriate treatment for individuals in the community.

  • Participate as a member of the Assertive Community Treatment multidisciplinary team.
  • Provide direct clinical services to include individual supportive therapy and psychotherapy to patients on an individual, group and family basis. Services provided by the ACT team are community based.
  • Provide individual and group treatment in a stage-based treatment model that is non-confrontational, considers treatment interactions of mental illness and substance abuse and has patient determined goals.
  • Develops individualized treatment plans to include patient-centered goals for treatment including both the patient and natural supports.
  • Develops individualized Crisis Plans that allow the patient to manage emerging crisis, and to identify resources for maintaining the individual in the community, to the greatest possible degree, during periods of crisis.
  • Facilitate and coordinate the individual’s treatment plan to assure maximum impact of the services and treatment provided.
  • Completes screenings of referrals to assess for appropriateness of patient to be admitted to ACT program and completes intakes as assigned.
  • May assess individuals for Revocation of Conditional Discharge from New Hampshire Hospital, as designated by the treating psychiatrist.
  • May assess individuals for Involuntary Emergency Admission to New Hampshire Hospital, as designated by the treating psychiatrist
  • Referral, follow up and monitoring of specialized treatment options as defined in the Individual Service Plan, and participate in the interdisciplinary teams of these providers to the degree possible. Coordinate with outside agencies and natural supports for continuity of care.
  • Participate in the delivery of treatment options including individual and group therapy, functional support services, and case management services
  • Assess effectiveness / efficiency of services provided to patients
  • Participates actively in clinical and administrative supervision. Attends and participates in daily team meetings to ensure effective communication among team members.
  • Provides on-call crisis intervention services covering nighttime hours with rotating weekend and evening shifts
Education and Experience:
  • Master's degree in psychology, social work, or related human services field
  • 2 years’ experience in community mental health preferred
  • Must have a valid driver's license.
Physical Requirements:
  • Is able to sustain light physical effort such as frequent standing, walking, bending, stooping, or reaching and/or occasional lifting of items 20 lbs. or more.
  • Is able to tolerate periods of heavy workload or excessive stress.
  • Must be able to walk, climb and use stairs on a regular basis and in unknown circumstances.
