Position Overview

Location: Sunland, Los Angeles

Job Summary: Provides evidence-based screening, psychosocial assessment, and brief intervention treatment as part of the healthcare team. Offers clinical services, mental health prevention, and case management for patients and families, integrating medical diagnoses, social determinants of health, and behavioral health issues.

Responsibilities

• Assess mental health issues of patients referred by clinic PCPs.
• Conduct clinical work with diverse patient populations of all ages.
• Use evidence-based screening tools; develop person-centered treatment plans.
• Assess motivation for substance use and mental health treatment; engage or refer patients accordingly.
• Provide short-term clinical interventions to support patient goals.
• Offer mental health prevention services for at-risk patients.
• Perform case management as needed.
• Document assessments, interventions, and treatment plans following guidelines.
• Participate in multidisciplinary teams, promoting integration between primary care and behavioral health.
• Maintain patient records and community resource relationships.
• Engage in supervision and professional development.
• Supervise social work interns if eligible.

Additional Duties for LCSW

• Supervise MSW interns and oversee their services.
• Conduct intake and screening of new patients.
• Facilitate training sessions and groups.

Requirements

ACSW (Associate Clinical Social Worker)

• Master’s degree in social work from an accredited university; two years post-master’s preferred.
• Registered with BBS, working toward licensure.
• Bilingual in English/Spanish required.

LCSW (Licensed Clinical Social Worker)

• Relevant healthcare experience preferred; two or more years in a healthcare setting.

Compensation and Benefits

• ACSW: $70K-$75K annually.
• LCSW: $79K-$83K annually.
