Duly Health and Care is seeking a licensed clinical social worker to lead ACP conversations, advance directive facilitation, and hospice referrals within a multidisciplinary team. You will document in Epic and coordinate with primary care and care teams.

Requires IL licensure and at least 2 years in healthcare social work, with strong motivational interviewing and patient engagement skills. The role offers comprehensive benefits and a focus on compassionate, patient-centered care.

Qualifikationen

• Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) license in Illinois.
• At least 2 years of clinical social work experience in a healthcare setting.
• Experience conducting serious illness, end-of-life, or goals of care conversations.
• Strong patient engagement and active listening skills; able to work with a multidisciplinary team.

Aufgaben

• Conduct a minimum of 8 ACP conversations per day across telephonic, in-person, and care team channels.
• Manage an active ACP outreach panel with consistent follow-up.
• Document all ACP conversations and directives in Epic.
• Guide patients through completion of Advance Directives and Living Wills.
• Identify patients for hospice or palliative care referrals and educate on benefits.
• Escalate complex cases to ACP Advanced Practice Providers as needed.
• Collaborate with Care Allies, Care Coordinators, and PCPs.
