Job summary

Memorial Hermann Health System is seeking an ACO Quality Specialist to lead ongoing data collection and management for quality initiatives in the ACO population. You will align payer data with outreach programs to achieve contracted payer performance metrics and enhance overall quality outcomes.

The role emphasizes data integrity, collaboration with care teams, and robust outreach to patients and physicians to close gaps in care and improve performance metrics within Memorial Hermann's quality

Qualifications

• Three or more years of healthcare-related experience with data support.
• Experience working directly with patients, physicians and care teams.
• Strong computer skills including word processing and Excel; basic database knowledge.
• EMR software experience preferred.
• Excellent oral and written communication skills.
• Ability to manage multiple tasks and priorities with strong communication.

Responsibilities

• Be a member of the ACO/PHSO Quality team and support population groups.
• Provide data and resources to clinicians to reach quality goals.
• Assist patients and physicians in addressing gaps in care per payer contracts.
• Conduct outreach to patients and physicians to meet metrics.
• Manage payer files and Excel worksheets to track outcomes.
• Conduct claims research and audit medical records for performance.
• Participate in special projects and ownership of initiatives.
• Ensure adherence to policy, safety and quality standards.
