Provider Partners is seeking an ACO Partner Performance Manager to serve as the primary performance and relationship resource for partner practices and facility operators involved in ACO REACH/LEAD and MSSP. You will translate financial, utilization, clinical, quality, attribution, and risk adjustment data into practical strategies that improve partner performance and care delivery.

The role requires coaching across clinical operations, analytics, coding, and revenue cycle management to achieve

Qualifications

• Three years of direct experience supporting ACO REACH or MSSP.
• Proven ability coaching partner practices to performance improvements.
• Strong analytical skills with Medicare claims and risk data.
• Excellent written and verbal communication across clinical and admin audiences.
• Knowledge of value-based care and attribution concepts.

Responsabilités

• Serve as day-to-day contact and performance advisor for partners.
• Understand partner structures, care settings, and barriers.
• Define shared performance expectations and communication cadence.
• Identify concerns early and facilitate resolutions with recommendations.
• Translate data into prioritized improvement plans with milestones.
• Coach leaders on care coordination and operational execution.
• Monitor progress and revise plans as needed.
