Millennium Physician Group - Case Manager

Millennium Physician Group is seeking to hire a full-time Case Manager to join the ACO team. We are looking for a positive, energetic, well-organized individual who can multitask and think outside of the box. Our goal is to ensure an excellent patient experience.

The Case Manager is responsible for providing services that assist patients in achieving their highest level of wellness. The role focuses on client-centered services that link clients and their families with healthcare and community-based services, ensuring timely access to appropriate levels of care and support. Responsibilities include assessment, planning, coordination, advocacy, education, monitoring, and accurate documentation across program areas. The ideal candidate will demonstrate knowledge of growth and development and have the ability to assess, communicate, and care for various age-specific patient populations.

Required Knowledge and Experience

• Registered Nurse with an active and clear license in the State of Florida (must be maintained during employment)
• 3 years of clinical experience, with at least 1 year in case management preferred
