Description And Responsibilities

Adult Community Clinical Services is a Department of Mental Health funded service that is designed to deliver evidence-based interventions within a clinically focused model. Individuals served are diagnosed with mental illness or dual diagnosis and reside in their own homes, apartments, and in group living environments. Our services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals. The teams are tight knit, supportive and multidisciplinary. Open Sky’s talented clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff provide quality, compassionate care to the individuals served.

Salary

USD $22.07/Hr.

Description And Responsibilities

Individuals served are diagnosed with mental illness or dual diagnosis and reside in their own homes, apartments, and in group living environments. Our services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals. The teams are tight knit, supportive and multidisciplinary. Open Sky’s talented clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff provide quality, compassionate care to the individuals served.

The Deaf and Hard of Hearing Counselor will provide support to people who are either deaf or hard of hearing and are diagnosed with a mental illness and/or substance use disorder and live on their own, or with families, in the community. They will act as a liaison between Persons served, community agencies, and families to coordinate treatment including employment and education entities, Care Coordination Entities, Emergency Service Providers, and outpatient clinics. Outreach Counselors encourage and empower individuals to see beyond and live beyond perceived limitations by providing them with the tools required for success.

Other Key Responsibilities

  • Deliver interventions identified on the Treatment Plan with supervision from licensed clinical staff.
  • Implement activities that create opportunities for the development of valued roles and personal relationships in the community
  • Provide side-by-side support and coaching to help individuals socialize. Conduct both 1:1 and group activities geared toward increasing Persons’ readiness to engage in rehabilitation.
  • Ensure individuals have adequate financial support through assisting with finding gainful employment or applying for entitlements.
  • Participate in the On-Call rotation.

Qualifications

  • Applicant must be a part of the Deaf/Hard of Hearing community. Fluency in American Sign Language required.
  • High School Diploma, GED or equivalent required.
  • Valid Driver’s License, reliable transportation and acceptable driving record required.
  • Reliability, willingness to learn, and being open to new opportunities.
