Job summary

JobRx, Inc. in the Southeast United States is seeking a Pharmacy Technician to obtain and update medication histories for admitted or pending patients, ensuring accurate reconciliation in the EMR system.

You will collect detailed information from patients, healthcare providers, and pharmacies, and document both histories and allergies promptly. You will refer unresolved issues to pharmacists, maintain patient confidentiality, and assist with training technicians and staff on medication history

Qualifications

• PTCB certification required unless currently enrolled in PharmD program.
• State Board of Pharmacy registration required.
• 6 months to 1 year of experience as a pharmacy technician required.
• Preferred: 2 years of retail pharmacy experience.
• Ability to read physician notes, medication labels, and lists accurately.

Responsibilities

• Identify and complete medication histories from EMR for admitted or pending patients.
• Interview patients and collect medication information from providers, pharmacies, and care facilities.

Summary:

The specialist will prioritize medication histories, gather comprehensive medication information from multiple sources, and document details thoroughly to support clinical care.

Key Responsibilities:

• Navigate electronic medical records to identify and complete medication histories for admitted or pending patients.
• Collect detailed medication information through patient interviews, healthcare providers, pharmacies, and other care facilities.
• Compile accurate medication lists including drug names, dosages, routes, frequencies, PRN indications, and last doses taken.
• Document medication histories and allergy information promptly and accurately in the electronic system.
• Refer unresolved medication issues to pharmacists and admitting providers.
• Maintain patient and staff confidentiality by securing records and releasing information only to authorized personnel.
• Report safety hazards promptly to supervisors or appropriate personnel.
• Conduct educational sessions for technicians, nurses, and other healthcare staff, including patient education.
• Precept and train pharmacy technicians, students, pharmacists, and nursing staff on medication history processes.
• Assist with data collection for performance improvement initiatives.
• Perform other duties as assigned.

Education:

• Pharmacy Technician Certification Board (PTCB) certification required unless currently enrolled in a PharmD program.
• Registration with the state Board of Pharmacy required.

Experience:

• Manual dexterity for operating pharmacy equipment such as computers, phones, fax machines, and automated dispensing cabinets.
• Ability to stand and walk for extended periods (6-10 hours) and lift moderate loads (up to 10 lbs), including stooping and kneeling.
• Effective verbal and written communication skills are essential.
