Job Summary

The Community Coach for Thrive Intensive Community Wrap Around Supports Program provides high-quality services to individuals served by BHN. These services are part of a plan designed by the Thrive clinical team. This position is based in Northampton, MA, with travel required within the greater Pioneer Valley.

Essential Job Functions
  • Provides 1:1 direct services to enrolled individuals to teach coping skills, self-advocacy, job readiness skills, and public safety.
  • Assists individuals in developing social, community, and independent living skills, including personal hygiene, home maintenance, and activities of daily living.
  • Implements behavior plans developed by the supervising behavioral professional (Positive Behavior Supports Clinician).
  • Models the implementation of strategies for parents/caregivers.
  • Monitors plan implementation and reports progress to the team.
  • Coordinates with other service providers regarding the individual’s progress.
  • Participates in team meetings and collaborates to support service goals.
  • Documents daily interactions and contacts with other providers, focusing on specific goal areas.
Qualifications
  • Bachelor’s Degree and at least five (5) years of experience working with individuals with autism spectrum disorder (ASD) and serious mental illness (SMI).
  • Must be mobile, trained in MAP or willing to be trained to administer medications.
  • Knowledge of behavior analysis, behavior planning, and implementation of behavior plans is preferred. Willing to train as a Registered Behavior Technician.
  • Valid driver’s license, clean driving record, and reliable vehicle required.
  • Must be over 21 years old.
  • Ability to work with clients from diverse backgrounds.
  • Excellent verbal and written communication skills.
Additional Information

This is a mid-senior level, full-time position in the mental health care industry, with no supervisory responsibilities.
