Little Village Nursing and Rehabilitation Center is seeking a dedicated and compassionate LPN or RN to join our team in Chicago, IL. This 106-bed skilled nursing and psychiatric facility offers a full-time, 2nd shift position with a focus on high-quality resident care.

You will collaborate with interdisciplinary teams, maintain detailed records, and support residents' physical, emotional, and psychological well-being in a safe environment.

Qualifications

• High school diploma or equivalent.
• 1 year of experience as an LPN or RN.
• Current LPN or RN license in Illinois and BLS certification.

Responsibilities

• Apply nursing policies and procedures in resident care.
• Direct day-to-day nursing assistant functions and ensure policy compliance.
• Provide care addressing residents' physical, emotional, and psychological needs.
• Maintain accurate records of resident care and medication administration.
• Participate in quality assurance and facility surveys.
• Collaborate with interdisciplinary teams to develop care plans.
• Ensure a safe, clean environment for residents and staff.
• Perform nursing duties including medication administration, wound care, and rehab programs.
