Job summary

Community Health Services Campus in Prairie du Chien, WI seeks a Charge Nurse to oversee patient care and uphold clinical standards for residents in our skilled nursing facility.

This role involves supervising staff, coordinating assessments, administering medications, documenting care, educating patients and families, and collaborating with a multidisciplinary team to improve outcomes; base shifts 2pm–10pm with shift differentials.

Qualifications

• Active state nursing license (RN or LPN) and eligibility to work in Wisconsin.
• Experience in long-term care or skilled nursing preferred.
• Ability to supervise nursing staff and coordinate resident care.

Responsibilities

Summary

As a Charge Nurse at Community Health Services Campus, you will play a vital role in overseeing patient care and ensuring high standards of clinical practice. 2pm - 10pm Hours Open

Base Wage (without shift differentials): $36.06 - $45.56/hr (RN) and $28.95 - $36.00/hr (LPN)

+ 2nd shift differential: $5.30/hour

+ Weekend shift differential: $4.75/hour

Responsibilities
