Overview

Responsibilities
  • Under the direction of a Coordinator or Supervisor within the Life Consultation Unit of CSDD, provides community-based direct and indirect support to individuals to create and implement a self-directed arrangement.
  • Support people within their person-centered plan, including engaging with the person’s family and friends and Case Manager within the utilization of Medicaid funded supports.
  • Treat to need and support people through a variety of situations and different environments.
  • Consult with people and their teams, perform collateral activities on behalf of the person, maintain detailed documentation, create budgets, and provide training.
  • Carry out all activities of the program/sub-unit in a manner that fulfills CMHA-CEI’s mission, policies and procedures.
Qualifications
  • Bachelor’s Degree in Social Work, Psychology, or closely related field.
  • Michigan credential (e.g., LLBSW, LBSW, SSTR) required.
  • One year of professional work experience with persons with developmental disabilities required.
  • Ability to communicate accurately and effectively in writing and verbally.
  • Valid Michigan driver’s license and access to reliable transportation for job-related use.
  • Ability to operate a computer and gain proficiency in Care Manager, the agency software for service authorizations, service entry, and billing for Contract Providers.
  • Physically capable of interacting with and/or transporting consumers of various height and weight.
  • Ability to tolerate exposure to body fluids and risk of consumer aggression.
  • May be required to be fingerprinted.
Conditional Employment Requirements
  • Background verification and credentialing procedures: Michigan State Police clearance check, verification of a professional license, Medicaid/Medicare verification, verification of an acceptable driving record, educational background check, and verification of an acceptable work history.
  • Must undergo and pass a Central Registry Clearance Check.
  • Must pass a pre-employment 5-panel drug screen.

Salary/Hours

Starting salary $24.04-$27.33 hourly, 40 hours per week. Must have ability to maintain a flexible work schedule to include some evenings and weekends as required per the needs of the caseload.

Location

Community Services for the Developmentally Disabled, Lansing, Michigan. This position offers the potential of a hybrid/alternative worksite assignment for community-focused supports.
