Job summary

Optum Home & Community Care is seeking a Nurse Practitioner to join our HouseCalls team in Tulare County, CA. In this role, you'll conduct in-home health assessments that identify care needs, close gaps in care, and connect patients to resources, all while building meaningful one-on-one relationships.

Required Qualifications

• Active and unrestricted Nurse Practitioner license, or the ability to obtain licensure in the state of assignment prior to start date.
• Current national certification through ANCC or AANP as a Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), Adult-Gerontology Acute Care Nurse Practitioner (AGACNP), or Gerontology Nurse Practitioner (GNP), or the ability to obtain certification prior to start date.
• Ability to perform the physical requirements of the role, including lifting and carrying equipment up to 30 pounds, navigating stairs, and conducting assessments in a variety of home environments.
• Comfortable with prolonged periods of standing, sitting, bending, and kneeling while completing patient assessments.
• Reliable transportation and willingness to travel locally to conduct in-home patient visits.
• Strong communication skills and a commitment to delivering patient-centered care in a home-based setting.

Pay range and compensation package:

The salary range for California residents is $109,500 to $164,000 per year. Pay is based on several factors including but not limited to education, work experience, certifications, etc. In addition to your salary, UnitedHealth Group offers benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
