Job Description
As a valued member of the Bluebonnet Community Services Youth and Family services team, this professional position conducts a comprehensive assessment of the youth and their families, working in full partnership with team members to develop a plan of care, oversee implementation, identify providers of services or family-based resources, facilitate child and family team meetings, and monitor all services authorized for a child's care.
The Wraparound Facilitator ensures that care is delivered in a manner consistent with strength-based, family-driven, youth-guided, and culturally competent values. They offer consultation and education to providers regarding these principles, monitor progress toward treatment goals, and ensure data for evaluation is properly gathered and recorded.
This position requires an aptitude for working with individuals with mental health diagnoses and knowledge of evidence-based practices in recovery promotion for severe and persistent mental illnesses. It offers opportunities for professionals skilled in assessment, collaborative person-centered recovery, differential diagnosis, and wellness and recovery-oriented systems of care.
Locations: Fayette and Lee Counties
Hourly Rate: $22.22 – $27.97/hour
Range of salary is subject to applicable differentials listed below:
Additional Incentives: $200/month Service Delivery incentive

Minimum Qualifications: Bachelor’s degree in a social, behavioral, or human service field or equivalent coursework, knowledge of mental illness effects, valid Texas Driver’s License.
Preferred Qualifications: One year of mental health assessment or crisis intervention experience, Master’s degree in psychology, social work, or related field, bilingual preferred.
Salary Range: $46,217 - $58,177/year
