Position Summary

The Case Manager is responsible for evaluating and directing the care and supervision of residents in Hope Harbor Center Licensed Residential Drug and Alcohol Treatment Facility. Responsibilities include developing and coordinating activities to meet Evidence-Based Practices, entering patients into HMIS, CAL-OMS, and DMC (Drug Medi-Cal), identifying needs, and collaborating with Clinical Program Manager and Counselors to ensure optimal treatment outcomes.

Essential Functions
  1. Engage and coordinate all scheduled In-Service Training classes.
  2. Maintain a caseload of 12-14 clients.
  3. Ensure clients adhere to house rules and log incidents.
  4. Conduct weekly case conferences with the program supervisor.
  5. Develop and implement Evidence-Based Practice curriculum following certification guidelines, working closely with the Clinical Program Manager.
  6. Review charts to maintain efficiency.
  7. Ensure all clients maintain current TB tests and physical exams.
  8. Report repairs to the Operations Manager and conduct random room checks.
  9. Follow emergency procedures and lead safety protocols in case of emergencies.
  10. Possess knowledge of DMC (Drug Medi-Cal), understand the six dimensions of ASAM, and be able to create treatment plans, conduct case management, and understand American Social Addiction Medicine.
  11. Manage all treatment services provided to clients at Hope Harbor Center.
  12. Develop and provide supportive counseling to clients and their families.
  13. Develop community outreach programs and oversee referral systems for mental health, medical/dental, vocational, and additional therapy needs.
  14. Network with community resources for referrals.
  15. Work with Veterans Per Diem Contracts, DMC, and IHP/DMH Grants.
  16. Perform additional duties as assigned by the Director or Clinical Program Manager.
Working Conditions

Ability to walk, stand, bend, squat, climb, kneel, and twist intermittently or continuously. Ability to grasp, push, pull objects such as files and operate a computer, fax, and telephone. Ability to lift up to 50 lbs.

Minimum Qualifications
  1. Must be a certified or registered Alcohol and Drug Counselor (AOD), with up-to-date registration.
  2. Bachelor's degree in social work/human services preferred.
  3. Experience working with parolees, mental health clients, DMC, and homeless populations is desired.
  4. Proficient in Microsoft Word and Excel.
  5. Valid California Driver's License with a clean MVR check.

  3. Ability to manage multiple priorities with attention to detail.
  4. Knowledge of Salvation Army policies and procedures is preferred.
  5. Criminal background check and certification for Protect the Mission policies are required if working near children.
