Position Overview

Aveanna Healthcare has grown tremendously during the past year through a series of exciting acquisitions, and we are currently looking for a Physical Therapist (PT) to join our dynamic home health care team. In this position, you enable patients to recover from illness or injury in the comfort of their own home.

Schedule: Monday - Friday, Flexible visit times

Essential Job Functions

• Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function.
• Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes.
• Provide written evaluations and plans of care outlining physical therapy needs, goals, progress, and expected outcomes.
• Deliver physical therapy services in accordance with physician orders.
• Maintain plans of care through timely documentation, modifying goals and treatments as needed, and planning discharges.
• Participate in discharge planning, including educating patients and caregivers on home exercise plans.
• Coordinate services with healthcare team members, physicians, and family.
• Document according to policies, requiring minimal supervision.
• Ensure documentation is effective to prevent technical or clinical denials.
• Maintain effective communication with staff, healthcare team, and family; serve as a resource for clinical staff.
• Supervise other personnel (PTA, HHA) as applicable.
• Complete OASIS assessments where permitted by state regulations.
• Follow policies, procedures, and conform to all patient care standards, licensure, and legal requirements.

Requirements

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license as a Physical Therapist.
• Valid CPR certification.
