Job summary

THOM Thomas Hospitals is seeking a patient care coordinator to provide one-on-one guidance, support, education, referrals, and coordination of care as patients move through the healthcare continuum.

You will develop relationships with physicians and service providers, liaise with medical staff, guide patients to appointments, and coordinate logistics for plan adherence while supporting patient families in a compassionate, collaborative setting.

Qualifications
