Job summary

Qualifications

• Background checks and DMV record checks required.
• Experience with individuals with disabilities preferred.
• 1:1 support and community integration experience is valued.

Responsibilities

• Provide 1:1 mentoring and support to help the individual achieve goals at home and in the community.
• Log daily times and service documentation accurately.
• Assist with activities of daily living and transportation as needed.
